Petrous Apex Lesions

چکیده
Situated lateral and adjacent to clivus. Petrous apex bisected by Internal auditory canal in coronal plane Anteriorly into peritubal area and posteriorly into perilabrynthine area. Anterior to the internal auditory meatus that is wedged between the posterior border of the greater wing of the sphenoid and the basilar part of the occipital bones Its borders and boundaries are marked by major blood vessels and structures, the superior and inferior petrosal sinuses above and below, and the horizontal segment of the internal carotid artery (lCA) and Eustachian tube laterally. The apex forms the posterolateral boundary of the foramen Lacerum. Apex is preformed in cartilage. The trigeminal ganglion lies in a depression on its posteromedial aspect and the abducens nerve crosses its superior surface beneath the petrosphenoidal ligament. Degree of pneumatization is variable (Peritubal area frequently pneumatised )

Petrous apex lesions of temporal bone progress slowly. Most of the time not only destruct this area but also involve neighbouring element. The symptoms of the neighbouring neuro-vasculare involvement we can recognize these lesions. The most common symptoms of involvement of the petrous apex are: headache, conductive hearing loss or sensorineural type, paresthesia and anesthesia of the trigemina...
متن کاملCASE REPORT Lobulated Petrous Apex Cholesterol Granuloma Treated with Infracochlear and Transsphenoidal Approaches
404 Petrous apex cholesterol granulomas are relatively rare lesions. They are usually unilocular and treated with simple fenestration. However, lobulated lesions require total removal of debris inside the cysts. Herein, we report a first case with a lobulated petrous apex cholesterol granuloma. The lesion extended from cerebello-pontine angel to the clivus. It was lobulated and contained debris...
متن کاملA novel endoscopic technique for long-term patency of cholesterol granulomas of the petrous apex.
INTRODUCTION Cholesterol granulomas (CGs) are rare benign cystic lesions containing cholesterol crystals in a gluelike fluid and are frequently found in the petrous apex (PA) and in the nearby anatomical area. The gold standard treatment of PA CG counts on its surgical drainage and ventilation to prevent recurrence. Different surgical approaches have been described in the past to achieve draina...

PURPOSE When troublesome MR imaging findings are noted in the petrous apex, the radiologist must determine if the area in question needs surgical therapy. Two nonsurgical entities, asymmetric fatty marrow and fluid-filled petrous air cells (trapped fluid), can be noted on conventional brain MR images and confused with pathologic lesions. Our observation that radiologists do not always confident...
متن کاملCombined lateral microscopic/endoscopic approaches to petrous apex lesions: pilot clinical experiences.
BACKGROUND Surgical treatment of lesions involving the temporal bone, petrous apex, or internal auditory canal is usually performed using the classical microscopic approach that necessitates wide external incisions and soft tissue dissection. At present, the main application of endoscopic surgery is in the surgical treatment of middle ear cholesteatoma, but with the natural evolution of the tec...
